Position Summary
Our dedicated Chronic Care nursing team make a lasting difference in the lives of patients with complex, long-term medical needs. Key Responsibilities
- Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.
- Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.
- Monitor, assess, and document patient progress while ensuring safety and comfort.
- Educate patients and families on treatment plans, medication management, and discharge planning.
- Support a culture of excellence, teamwork, and continuous learning.
Qualifications
- Required: Current Registered Nurse (RN) license in the State of Connecticut.
- Required: Graduate of an accredited school of nursing (BSN preferred).
- Required: Demonstrated competence in the clinical setting.

COMPENSATION & BENEFITS
At Hospital for Special Care, we are committed to supporting the health, financial well-being, and professional growth of our team members. The wage range for this position is $38.50 to $57.82.
